The most common post-acute care is paid, regulated long-term care, which encompasses a range of supportive, rehabilitative, nursing, and palliative services provided to people—from young to old—whose capacity to perform daily activities is restricted due to chronic disease or disability. There are five major sectors of long-term care:

· Adult day services centers
These are safe, professional care settings for adults who need supervision outside the home during the day. They serve older adults, adults with dementia, and adults with disabilities requiring individualized therapeutic, social, or health services during part of the day. They provide social and some health services in community-based group settings.

· Home health agencies
Home health care is provided to individuals and families in their place of residence to promote, maintain, or restore health or to maximize the level of independence.

· Hospice
Hospice care is defined as a program of palliative and supportive care services that provides physical, psychological, social, and spiritual care for dying persons, their families, and other loved ones. Hospice services are available in both the home and inpatient settings.

· Nursing home
Nursing homes, also called skilled nursing facilities, provide a wide range of health and personal care services. Their services focus on medical care more than most assisted living facilities. These services typically include nursing care, 24- hour supervision, three meals a day, and assistance with everyday activities. Rehabilitation services, such as physical, occupational, and speech therapy, are also available. Some people stay at a nursing home for a short time after being in the hospital. Others reside there permanently because of ongoing conditions.

· Residential care communities
Residential care communities provide care and supervision for people who are unable to live by themselves but who do not need 24-hour nursing care. They are considered nonmedical facilities and are not required to have nurses, certified nursing assistants, or doctors on staff. Other terms used to refer to this level of care are assisted living facilities and Continuing Care Retirement Communities that provide personal care and supervision.
